29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Basement Electrical Service Room: The employer failed to instruct each employee in the recognition and voidance of unsafe conditions where the employees were directed to work in an electrical service room with voltage of approximately 4kv. This exposed employees to electric shock and burn hazards.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: Basement Electrical Service Room: The employer directed employees to work in close proximity to electric power circuits with an approximate voltage of 4kv without protection from electric shock by de-energizing or other protective means. This exposed employees to electric shock and burn hazards where an employee suffered serious life threating injuries.
