29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings and other structures: Location: 195 Sullivan Place, Brooklyn, NY. a) Accumulated construction debris and materials on the ground were causing tripping hazards to employees. Employees fell on debris and materials. On or about 08/10/2017.

29 CFR 1926.756(a)(1): 29 CFR 1926.756(a)(1): During the final placing of solid web structural members, the load was released from the hoisting line before the members were secured with at least two bolts per connection, of the same size and strength as shown in the erection drawings, drawn up wrench-tight or the equivalent as specified by the project structural engineer of record: Location: 195 Sullivan Place, Brooklyn, NY. a) Employees who were preparing to secure a steel beam and it was release from hoisting line and it fell knocking the employees from the ladder. On or about 08/09/2017.
