29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On or about August 10, 2017, the employer did not ensure that employees safely arranged a platform of an industrial scissor lift on the forks of a fork truck when using the forks for the purpose of lowering the platform from its elevated position to the ground in order to decommission the scissor lift.

29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area: a) 350 Lowell Street, Andover MA 01810, Hampshire Building. On or about August 10, 2017, the employer did not ensure that all authorized and affected employees had been safely positioned or removed from the work area prior to removing lockout or tagout devices and restoring energy to the machine. A scissor platform lift was in the process of being decommissioned and removed. After an authorized employee removed stop-block devices from the lift, the employee released a strap used to remove the devices, causing the lift to strike the employee, injuring the employee. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
