29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area:  a) 350 Lowell Street, Andover MA 01810, Hampshire Building.   On or about August 10, 2017, the employer did not ensure that all authorized and affected employees had been safely positioned or removed from the work area prior to removing lockout or tagout devices and restoring energy to the machine. A scissor platform lift was in the process of being decommissioned and removed. After an authorized employee removed stop-block devices from the lift, the employee released a strap used to remove the devices, causing the lift to strike the employee, injuring the employee.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
