Employees were observed operating the TRAK DMP3 milling machine in the maintenance area with rotating parts that was not guarded to prevent the employee from being pulled into the work area.

In the facility there were a couple of instances where cord and cables were not provided strain relief.     a) In the shipping area employees were using a portable fan that had exposed wiring from failed strain relief.      b) In the fabrication area employees were using a portable hand grinder that had no visible strain relief.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Employees were exposed to electrical shock hazards associated with cord a plug equipment. In the facility there were several instances where cord and plug equipment was missing a ground pin.      a) In the ramp and carrier area a portable pedestal fan was in use and was missing its ground pin.    b) In the shipping area a portable fan was in use and was missing its ground pin.    b) In the fabrication area portable hand grinder was in use and was missing its ground pin.

29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not netralized, power was not shut off, and brakes set.  Employees were exposed to struck-by hazards. This was demonstrated when the employer failed to protect employees from being struck by a powered industrial truck that was left unattended and the parking brake was not set.
