29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment; a. On or about August 03, 2017, in the fab shop area, an employee performed drilling operations on the Bridgeport mill, serial #J-69067, was wearing Hyflex gloves, type 11-510, that were supplied by the employer to protect against burn hazards. The selection and use of gloves during the operation was inappropriate as they did not protect against the more severe caught-in hazards of the rotating chuck and cutting bit, and increased the probability of injury from them.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about August 03, 2017, in the fab shop area, the Bridgeport mill, serial #J-69067, did not have the rotating chuck and cutting bit guarded from accidental contact, exposing an employee to a caught-in hazard.
