29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:    a) Plant - On or about 08/14/17 and at times prior thereto the employer operated the (older) Hantscho printing presses without guards. Workers were exposed to accidental contact with the rollers.    c) Plant - On or about 08/14/17 the employer operated the stapler/stitcher machine, and the interlock at the door of the stitcher head was not operating.     Or in the alternative:    29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):      a) Plant - On or about 08/14/17 and at times prior thereto the employer operated the (older) Hantscho printing presses without guards. Workers were exposed to accidental contact with the rollers.      c) Plant - On or about 08/14/17 the employer operated the stapler/stitcher machine, and the interlock at the door of the stitcher head was not operating.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii):    a) Plant - On or about 08/14/17 and at times prior thereto the employer operated the Stitcher/Stapler machine, and the gear pair at the unused pocket was not guarded.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:      On or about 08/14/17 and at times prior thereto employees worked in proximity of the Hantscho printing presses and the dryer, and the employer did not assess the level of noise in order to determine whether a Hearing Conservation Program was required. (Instantaneous noise measurements: 90.7 dB, 90.4 dB, 97.3 dB, and 92.5 dB).    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
