29 CFR 1926.251(a)(6): Each day before being used, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person designated by the employer: JOB SITE: 4100 Forest Park Avenue, Saint Louis, MO 63110, on or about 8/14/2017, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person designated by the employer. A shackle used to connect the wire rope on a suspended scaffold was defective and was not removed from service. The workers were exposed to a fall hazard.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: JOB SITE: 4100 Forest Park Avenue, Saint Louis, MO 63110, on or about 8/14/2017, workers were observed using a yellow flexible extension cords where the path to ground from circuits, equipment, or enclosures was not permanent and continuous. The cord was used in the suspended scaffold to power a box radio and was missing the ground prong on the male end of the cord. The workers were exposed to an electrical hazard.
