29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) On or about August 11, 2017, employees were exposed to injuries resulting from falls while accessing the roof of a commercial building from a JLG Industries, Inc., model # 600S, serial # 0300233949, aerial lift with an articulating boom without the benefit of a fall restraint system being in place.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) On or about August 11, 2017, employees were exposed to injuries from falls, while conducting roofing activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.
