This was most recently documented at 1405 Norman Ave, Crete, NE 68333 inside building 076 on or about August 10, 2017 and at times prior when employees accessed the unprotected side of the dock without the use of fall protection to load trucks with waste and dust sweepings from the mill.

This was documented on or about August 10, 2017 and times prior in the maintenance shop located in building 050 at 1405 Norman Ave, Crete, NE 68333 when employees operated the drill press without guarding to protect them from the rotating chuck.      Instance C:  The employer is failing to protect employees from entanglement hazards when operating the Wilton VSG Twenty drill press.  This was documented on or about August 10, 2017 and times prior in the maintenance shop located in building 050 at 1405 Norman Ave, Crete, NE 68333 when employees operated the drill press without guarding to protect them from the rotating chuck.      Instance D:  The employer is failing to protect employees from entanglement hazards when operating the Walker Turner drill press.  This was documented on or about August 10, 2017 and times prior in the tin shop located in building 050 at 1405 Norman Ave, Crete, NE 68333 when employees operated the drill press without guarding to protect them from the rotating chuck.
