29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a) Christie Lodge Owners Association at 47 E Beaver Creek Blvd., Avon, CO 81620: On or about 8/11/2017, the employer did not ensure that each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports. Three employees were utilizing the scaffold in order to replace supporting trusses underneath condo decks on the first, second, and third floors. The second and third levels of the scaffold were not fully planked or decked. This condition exposed employees to fall hazards in of approximately 10 and 15 ft.

29 CFR 1926.451(e)(9)(iv): The employer did not ensure that cross braces on tubular welded frame scaffolds were not be used as a means of access or egress:    a) Christie Lodge Owners Association at 47 E Beaver Creek Blvd., Avon, CO 81620: On or about 8/11/2017, the employer did not ensure that cross braces on tubular welded frame scaffolds were not be used as a means of access or egress. Three employees were utilizing the scaffold in order to replace supporting trusses underneath condo decks on the first, second, and third floors. An employee was observed climbing onto the scaffold via cross braces on the third floor. This condition exposed the employee to fall hazards in excess of approximately 15 ft.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level:  a) Christie Lodge Owners Association at 47 E Beaver Creek Blvd., Avon, CO 81620: On or about 8/11/2017, the employer did not ensure that each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.  Three employees were utilizing the scaffold in order to replace supporting trusses underneath condo decks on the first, second, and third floors. The scaffold was missing mid rails on all levels and sections and an employee was documented on the third level of the scaffold without fall protection. This condition exposed employees to fall hazards of approximately 10 and 15 ft.
