29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  A) Second floor area, on or about August 11, 2017, employees were working near the open sided edge of the second floor while installing the plywood decking onto the 2nd floor joists without using fall protection equipment, exposing employees to a fall hazard of approximately 11 feet and 14 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A) South side of building, on or about August 11, 2017, the extension ladder used by the employees to gain access to the second floor area did not extend three feet above the landing, exposing employees to a 14 foot fall hazard.
