Greystone, Sioux Falls SD, 57108:  On or about and at times prior to August 10, 2017, the employer did not ensure that each employee was protected from falls from heights in that two employees were installing a cover on the roof of a four story apartment complex without fall protection in place.  This condition exposed the employees to a fall hazard of approximately 40 feet.
