29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about August 14th 2017, at House # 9/8 1641 in Turquesa by Lennar, located at NW 184 St and 89th Avenue Hialeah FL 33015,  two employees were observed working on a deck approximately 8 feet from the ground and was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 14th 2017, at House # 9/8 1641 in Turquesa by Lennar, located at NW 184 St and 89th Avenue Hialeah FL 33015, two employees were accessing the working area of the deck with a wood ladder that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard of approximately 8 feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about August 14th 2017, at House # 9/8 1641 in Turquesa by Lennar, located at NW 184 St and 89th Avenue Hialeah FL 33015, employees were accessing the deck with a wood ladder, without being trained on how to use the ladder, exposing the employees to fall hazard of approximately 8 feet.
