29 CFR 1926.451(b)(4): Each end of a platform, unless cleated or otherwise restrained by hooks or equivalent means, did not extend over the center line of its support at least 6 inches (15cm): a) Employees were exposed to fall hazard on the tubular welded frame scaffold; several scaffold plank overhangs were too short, less than 6 inches. The plank can shift and fall from the scaffold frame. Location: 89 Bartlett Street, Brooklyn, N.Y. On or about 8/10/2017 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(b)(5)(i): Each end of a scaffold platform 10 feet or less in length shall not extend over its support more than 12 inches (30 cm) unless the platform is designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end: a) Employees were exposed to fall hazard on the tubular welded frame scaffold; scaffold plank overhangs were too long, up to 24 inches. An employee stepped on the end of s plank and it tipped-up causing the employee to fall 6 feet. Location: 89 Bartlett Street, Brooklyn, N.Y. On or about 8/10/2017 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
