29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: (a) Milling Department - At a Hwacheon Vista 1000 12K CNC milling machine, the blue air nozzle had the pressure reducing tip removed allowing full-line pressure to be used for blowing chips from the parts being machined. The air nozzle pressure tested exceeded 104 PSI. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Lathe Department, Nakamura-Tome A-200 CNC Turning Center, S/N KS-2001104 (NAK#2) - On or about August 15, 2017, an employee operated the lathe with the door safety interlock disconnected/bypassed. (b) Milling Department, Kitamura MyCenter Zero CNC Milling Machine, S/N 75400 (MY#8) - During the period between June 1, 2017 and August 22, 2017, an employee operated the mill with the door safety interlock disconnected/bypassed. (c) Milling Department, Kitamura CNC Milling Machine (MY#10) - During the period between June 1, 2017 and August 22, 2017, an employee operated the mill with the door safety interlock disconnected/bypassed. (d) Milling Department, Kitamura Thunder CNC Milling Machine (MY#5) - On August 22, 2017, an employee was operating the mill with the outer doors missing from the rotating pallet changer and the safety switch bypassed/tied back with plastic cable ties. (e) Milling Department, Mori-Seiki CNC Milling Machine, Model SH400, S/N 816 (MORI#2) - On August 22, 2017, an employee was operating the mil with the door safety lock disconnected. Schultes Precision Manufacturing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 676898, citation number 1, item number 2 and was affirmed as a final order on November 7, 2012, with respect to a workplace located at 1250 Busch Pkwy, Buffalo Grove, IL 60089. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) Throughout the facility - The employer's documented hazardous energy control procedures (Lockout/Tagout) procedures were deficient in that they did not specifically outline the scope, purpose means to enforce compliance and the magnitude(s) of the various energy sources encountered. On August 22, 2017, MyCenter Zero CNC Milling Machine (MY#3) was undergoing a maintenance activity and had been locked out by the Building Supervisor. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
