Employees were shingling a steep-pitch two-story single family home without the use of safety glasses. Condition exposed employees to the hazard of being struck-by nails and debris.

Employees were shingling a steep-pitch two-story single family home without the use of fall protection. Condition exposed employees to fall hazards of approximately 20 feet.
