29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:(a)(IDENTIFY LOCATION(S))(DESCRIBE CONDITIONS AND TYPE OF AERIAL LIFT) On or about August 15, 2017: a) Employees utilizing an aerial lift to gain access to an upper working area were exposed to a fall hazard of approximately 50 feet when the employer failed to ensure that the employee in the basket was attached to the boom or basket of the aerial lift. No abatement certification or documentation is required for this item.
