29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   On or about April 27, 2017, at the above addressed site, employees were exposed to a fall hazard of approximately 11 feet to 14 feet 6 inches when performing roof work on a low slope roof while not being protected from falling to the lower level.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about April 27, 2017, at the above addressed site, employees were exposed to a fall hazard of approximately 11 feet to 14 feet 6 inches when performing roof work on a low slope roof prior to being provided with a training program that would enable them to recognize the hazards of falling.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:   On or about April 27, 2017, at the above addressed site, employees were exposed to a fall hazard of approximately 14 feet 6 inches when gaining access to and from a roof by using an extension ladder that was not secured to prevent accidental displacement.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  On or about August 15, 2017, at the above addressed site, employees were using chemicals including but not limited to Liquid Propane Gas and Owens Corning Trumbull Type 4 Trulo Roofing Asphalt while the employer did not maintain the safety data sheets for those products at the worksite.
