29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    15 Leete St, Springfield , MA  On August 15 and August 16, 2017, employees were exposed to fall hazards of more than twenty feet while they were performing residential roofing work.  Employees left the ladder and climbed to the peak before they attached the lanyard to their fall protection harness.

29 CFR 1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials:  15 Leete St, Springfield, MA  On August 15, 2017, employees were exposed to fall hazards due to improper connectors of their personal fall arrest systems.  Some employee lanyards were connected by a tied knot to the anchor points and some lanyards were connected to the harness straps instead of the D-ring.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  15 Leete St, Springfield, MA  On August 16, 2017 and at times prior, the employer did not provide proper training to employees on the personal fall arrest systems they were using.  Employees did not know that some of their personal fall arrest system practices were prohibited.   Specifically, they allowed their harnesses to be loose and twisted, their lanyards to be connected to anchors using knots, their anchors to not be fully attached to roof by hardware, and their anchor fasteners to be those other than specified by manufacturer.
