29 CFR 1910.140(c)(22): Personal fall protection systems were not worn with the attachment point of the body harness located in the center of the employee's back near shoulder level.  The attachment point may be located in the pre-sternal position if the free fall distance is limited to 2 feet (0.6 m) or less:    On or about August 11, 2017, at the above addressed jobsite, employees were performing maintenance/repairs on street lights (replacing bulbs/worn out fixtures) from the basket of an aerial lift, wherein the employee was wearing a personal fall arrest system that did not meet the requirements in subpart I of this part, therefore exposing the employees to a fall hazard.    NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.30(a)(3)(iii): The employer did not train each employee in the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee used:    On or about August 11, 2017, at the above addressed jobsite, employees were performing maintenance/repairs on street lights (replacing bulbs/worn out fixtures) from the basket of an aerial lift, during which the employer did not provide training to each employee who is required by this section to use PPE, and wherein the employee was using PPE (PFAS), therefore exposing the employees to a fall hazard and/or suspension trauma/orthostatic shock hazard.      NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
