29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  On or about august 14, 2017, employees were working on the construction of a residential home. The jobsite was littered with rocks, debris and unused lumber. It was also observed that some of the pieces of unused wood had protruding nails on them. These conditions created not only a trip hazard for employees but a puncture hazard as well.

29 CFR 1926.403(b)(1)(iii): The employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees; sections of the electrical insulation on cords and cables was missing:   On or about August 14, 2017, employees were working on the construction of a residential home. They were using at least three extension cords that had damage to their insulation. This exposed the employees to electric shock hazards.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:  On or about August 14, 2017, employees were working on the construction of a residential home. They were using an extension cord that was missing the ground pin to power a Dewalt circular saw.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems or personal fall arrest systems:  On or about august 14, 2017, employees were working on the construction of a residential home. Two employees were observed working on the framework of the home without any form of fall protection. The employees were exposed to fall hazards of greater than six feet to the ground below.

Employees were exposed to fall hazards of more than six feet.
