29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:    On or about August 14, 2017, employees were working on the construction of several residential buildings. The jobsite was littered with unused lumber. It was also observed that some of the pieces of unused wood had protruding nails on them. These conditions created not only a trip hazard for employees but a puncture hazard as well.

The employee was exposed to fall hazards of more than six feet to the ground below.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:    On or about august 14, 2017, employees were working on the construction of several residential buildings. They were using four pieces of equipment that had damage to the insulation of their extension cords. They were also using at least two extension cords that had damage to their insulation and one extension cord that was missing the ground pin. This exposed the employees to electric shock hazards.     The four pieces of equipment are as follows:  1. Ridgid circular saw, Model R32021  2. Drill Master circular saw, item 63005  3. DeWalt reciprocating saw, Model DWE304  4. Skilsaw circular worm drive saw, Model MAG77LT
