29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) Residential property at 607 Bell Road, in Mount Ephraim, NJ, 08059, Front (Southeast) corner of the roof: Employer did not ensure that 120/240 volt service drop conductors were guarded against accidental contact or were de-energized prior to employees performing roofing work in immediate proximity, occurred and was observed on 8/16/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Residential property at 607 Bell Road, in Mount Ephraim, NJ, 08059, Front (Southeast) corner of the roof: Employees were exposed to an approximately 15 foot fall to the ground while performing residential roofing work without fall protection, occurred and was observed on 8/16/17. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. DJK ROOFING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1117795, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 5/18/2016, WITH RESPECT TO A WORKPLACE LOCATED AT 211 FIRST AVE, HADDON HEIGHTS, NJ, 08035.
