29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about August 16, 2017, the General Contractor's trailer had some strain relief damage to the extension cord that was used to power the trailer. This condition exposed employees to electrical shock hazards.

29 CFR 1926.405(g)(2)(v): Flexible cords and cables were not protected by bushings or fittings where passing through holes in covers, outlet boxes, or similar enclosures: On or about August 16, 2017, the General contractor's trailer power extension cord was run through a hole which had no bushing to protect the cord. This condition exposed employees to electrical shock hazards.
