29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:  a) Mud Belt Area- On or about August 10, 2017, the employer did not ensure that employees performing belt adjustments on the Big Machine Mud Belt Line understood the requirements for the safe application, usage and removal of the energy controls while accessing hazardous areas.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a) Mud Belt Area- On or about August 10, 2017, employees were exposed to machine hazards associated with moving parts and pinch points from the unexpected start-up and energization of the Big Machine Mud Belt Line when opening the guards that covered the conveyor belt and drive pulley.    The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform adjustments to the conveyor belt [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)],  dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.   No abatement certification or documentation is required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts:  a) Mud Belt Area- On or about Aug. 16, 2017, the employer failed to ensure that hazards created by the in running nip points and rotating parts on the Big Machine Mud Belt Line were guarded to prevent injury to employees during operation.  b) Mud Belt Area- On or about Aug. 16, 2017, the employer failed to ensure that hazards created by the in running nip points and rotating parts on the Little Machine Mud Belt Line were guarded to prevent injury to employees during operation.  No abatement certification or documentation is required for this item.

29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.  a) On or about August 10, 2017, a recordable injury occurred when an employee amputated his thumb.  The description in Column F did not describe the object that caused the amputation for calendar year 2017.   No abatement certification or documentation is required for this item.
