29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about August 16th 2017, at the construction site located at 3567 W 94th Ter. Aquabella by Lennar, House # 1/31 2409, Hialeah FL 33018, the employer did not provide fall protection training to an employee who was observed working on the top of a wall at approximately 10 ft. from the ground without any means of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about August 16th 2017, at the construction site located at 3567 W 94th Ter. Aquabella by Lennar, House # 1/31 2409, Hialeah FL 33018, employees were accessing the working area with a metal ladder that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard of approximately 10 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about August 16th 2017, at the construction site located at 3567 W 94th Ter. Aquabella by Lennar, House # 1/31 2409, Hialeah FL 33018, an employee was observed working on the top of a wall at approximately 10 feet above the ground, and was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.        The employer EBENEZER CARPENTRY, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1234486, citation number 1, item number 3 and was affirmed as a final order on 07/21/17, with respect to a workplace located at NW 74th Ter., House # 3/31, Grand Bay North project, Doral FL 33178.
