29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:   On August 29, 2017 located at 4515 East 139th Street, Grandview MO 64030 employees were exposed to noise hazards when the employer failed to implement a hearing conservation program. The action level calculated based on an 8 hour time weighted average resulting in above 85 dBA. The employer failed to provide a safe and healthy work environment free of hazards associated with noise levels.  (1) An employee working on Prier 1 line was exposed to continuous noise levels at 89.1% dose of the allowable 8-hour time weighted average sound level of 85 dBA or equivalent sound level of approximately 89.1 dBA. The sampling was performed for 459 minutes during one shift on August 29, 2017; exposure calculations included a zero increment for the 21 minutes not sampled.   (2) An employee working on Mansfield line was exposed to continuous noise levels at 59.1% dose of the allowable 8-hour time weighted average sound level of 85 dBA or equivalent sound level of approximately 86.2 dBA. The sampling was performed for 448 minutes during one shift on August 29, 2017; exposure calculations included a zero increment for the 32 minutes not sampled.  (3) An employee working on Mansfield line was exposed to continuous noise levels at 56% dose of the allowable 8-hour time weighted average sound level of 85 dBA or equivalent sound level of approximately 85.8 dBA. The sampling was performed for 456 minutes during one shift on August 29, 2017; exposure calculations included a zero increment for the 24 minutes not sampled.  (4) An employee working on Copper Cut line was exposed to continuous noise levels at 125.5% dose of the allowable 8-hour time weighted average sound level of 85 dBA or equivalent sound level of approximately 91.6 dBA. The sampling was performed for 450 minutes during one shift on August 29, 2017; exposure calculations included a zero increment for the 30 minutes not sampled.  (5) An employee working on Prier 1 line was exposed to continuous noise levels at 72.7% dose of the allowable 8-hour time weighted average sound level of 85 dBA or equivalent sound level of approximately 87.7 dBA. The sampling was performed for 464 minutes during one shift on August 29, 2017; exposure calculations included a zero increment for the 16 minutes not sampled.
