29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed: On or about August 17, 2017, three ladders were used to form a working platform that was set up and used on inside of the lift station, exposing employees to fall hazard up to approximately 10 feet.

29 CFR 1926.1203(e)(2)(ii): When entrance covers were removed the opening was not immediately guarded by a railing, temporary cover, or other temporary barrier that would prevent an accidental fall through the opening and that would protect each employee working in the space from foreign objects entering the space. On or about August 17, 2017, the lift station was opened and no guardrails were installed around the opening, exposing employees to a fall hazard of approximately 23 feet.
