29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or  similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers:    On or about August 17, 2017, the manhole cover was removed and no guardrails were installed around the opening, exposing employees to a fall hazard of approximately 13 feet.

29 CFR 1926.1051(a):  Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    On or about August 17, 2017, the employer did not provide a means of egress for one employee working in the bottom of the manhole at approximately 13 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.     On August 17, 2107, one half of a 28' extension ladder was lowered into a manhole approximately 13 feet deep and at least 3 feet did not extend above the upper landing, exposing one employee to a fall of approximately 13 feet.
