29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: At 3127 California Avenue, St. Louis, MO 63118, employee(s) were removing pre-existing shingles from the roof of a church without any form of conventional fall protection in place. The fall distance to the concrete sidewalk below was approximately 20 feet. The slope of the roof was approximately 12:12. This violation was observed on 08/17/17.
