29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. Location: 562 Grand St. On or about: 15 Aug 2018 a) Employees were working on scaffolding with the means to access it was to climb the ladder and then use the scaffold to get to the working platform that was a 10 ft. away from the ladder.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems: Location: 562 Grand St. On or about: 15 Aug 2018 a) Employees were working on scaffolding inside of the hoist area without a fall protection system. The employees were hoisting cement to the upper level of the scaffold and there was not a fall protection system in place.
