29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  On or about August 17, 2017:  a) Employee was exposed to eye injuries when using a pneumatic nail gun without the use of proper eye protection while using the nail gun to install wooden material.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 17, 2017:  a) An employee installing makeshift scaffold was exposed to a fall hazard of approximately 19.3 feet when the employer failed to ensure that the employee was using proper fall protection measures to reduce or eliminate the hazard that was present.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
