29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 17, 2017: a) Employees for Jose Ochoa were exposed to fall hazards from approximately 10.2 to 14.1 feet while installing floor sheathing on the second floor of a new residential structure. The employer failed to ensure that employees exposed to a fall hazard were properly trained to recognize and to minimize the fall hazard. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation. (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: On or about August 17, 2017: a) Employees were exposed to a fall hazard of approximately 10.2 feet while accessing a ladder system to gain access to the upper landing area of the work site, where the ladder did not extend the required three feet above the landing surface.
