29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a. On or about August 16, 2017 the employer had a bench grinder in front of the Mazak Machine Center # 368 that did not have the upper periphery guard installed on the grinder which caused the opening between the frame of the grinder and the abrasive wheel to exceed one-quarter inch, thereby exposing employees to struck-by hazards should the wheel break or shatter during operation. b. On or about August 16, 2017 the employer had a bench grinder by the tool bench for machines 363 and 365 that did not have the upper periphery guard installed on the grinder which caused the opening between the frame of the grinder and the abrasive wheel to exceed one-quarter inch, thereby exposing employees to struck-by hazards should the wheel break or shatter during operation. c. On or about August 16, 2017 the employer had a bench by the Mazak Machine Center # 361 (Model # Super Quick Turn 200MS) that did not have the upper periphery guard installed on the grinder which caused the opening between the frame of the grinder and the abrasive wheel to exceed one-quarter inch, thereby exposing employees to struck-by hazards should the wheel break or shatter during operation.

On or about August 16, 2017 the employer did not require employees to isolate the energy while performing blade changes on the #247 screw machine. G3 Industries was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.147(d)), which was contained in OSHA inspection number 1096089, citation number 1, item number 1b and was affirmed as a final order on November 12, 2015 with respect to a workplace located at 1405 Dons Way, Mosinee, WI 54455.
