29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems: On or about 16 August, 2017, at the above addressed jobsite, an employee was working on top of the form work on the home under construction without the use of fall protection, exposing the employees to a fall hazard of up to approximately 20 feet.
