29 CFR 1910.138(b): Employers did not base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified. This violation occurred on or about August 17, 2017, for areas such as, but not limited to, the ground beef production room, the employer had not adequately identified effective personal protective equipment such as tight fitting cold resistant gloves for employee protection while operating the patty forming machines exposing employees to caught in hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: This violation was observed on or about August 17, in the ground beef production room, the Hollymatic 580 patty forming machine, did not have the point of operation guarded exposing employees to the hazards of being caught-in the moving plate or struck by the stamping mechanism.
