29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about August 18, 2017, employees were exposed to a fall hazard of approximately 11.0 feet above the ground below while working off the roof of the Park United Methodist Church erecting the bricklayer's scaffold without the use of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 18, 2017, employees were exposed to a fall hazard of approximately 11.0 feet above the ground below while working off the roof of the Park United Methodist Church erecting bricklayer's scaffold without the use of fall protection and the employer did not have a training program for providing training to the employees.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: On or about August 18, 2017, an employee was exposed to a fall hazard while working from the bricklayer's scaffold because of the lack of fall protection. The employee working from the bricklayer's scaffold was exposed to a fall hazard of approximately 16.5 feet to the roof below.
