Spanish Fort, Alabama: On or about August 17, 2017 and at times prior; the employer allowed his employees to use an extension ladder to gain access to the trusses and the rails of the ladder did not extend at least three feet above the landing to provide safe access exposing his employees to a fall hazard of up to 12 feet 8 inches to the ground below.
