29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole: a) 25 Hilltop Road, Edison, NJ 08820. Employees were installing new flexible duct throughout the first and second floor of a newly constructed single family home while working in close proximity to unprotected interior first and second floor unprotected stairwells with no fall protections systems in place, i.e. guardrails.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) 25 Hilltop Road, Edison, NJ 08820. Employees were installing new flexible duct throughout the first and second floor of a newly constructed single family home while working in close proximity to unprotected interior second floor balcony and window openings with no fall protections systems in place, i.e. guardrails. Occurred on 08/19/17.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: a) 25 Hilltop Road, Edison, NJ 08820. Employee was exposed to fall hazards when he used a Werner 8 foot fiberglass "A" frame ladder to access the second floor balcony, 10 foot above the floor below, stepping on the most upper step resulting in the ladder not being used as intended. Occurred on 08/19/17
