29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels did not have guardrail systems which complied with subpart M: a) Job site, 1611 Davis Avenue, Pittsburgh, PA: On or about August 19, 2017, during construction activities, the employer did not ensure that the ramp leading from the main building onto the scaffolding where the scrap chute was located had appropriate fall protection in place in that there was only a top rail in place, leaving openings on both sides of the ramp which were determined to be approximately 28 inches wide by 42 inches tall, exposing employees to fall hazards of up to approximately 30 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level: a) Job site, 1611 Davis Avenue, Pittsburgh, PA: On or about August 19, 2017, during construction activities, the employer did not ensure that employees were provided proper fall protection on the dump chute scaffolding in that the midrail section along the front of the scaffold was missing, leaving an opening determined to be approximately 42 inches tall and 84 inches wide, exposing employees to fall hazards of up to approximately 30 feet.
