At the worksite - On or about August 18, 2017, employees used the top cap and top step of a Werner stepladder to gain access to the structure's top plate to install the second floor joists, thus exposing employees to a fall hazard.

At the worksite - On or about August 18, 2017, the employer did not provide ladder training that included elements such as, but not limited to the proper usage of a Werner stepladder, thus exposing employees to a fall hazard.
