OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm, in that employee(s) were exposed to fall hazards within the elevator shaft: (a) On August 15, 2017, an employee fell approximately 20 feet into an elevator shaft from the main lobby on the first floor to the elevator pit floor below. The hoistway door was opened with a hoistway door key and the elevator car was not at the landing.

The employee was confirmed deceased at the scene on this date.
