29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:      a)  Rear Parking Lot - On or about 08/17/2017, workers were exposed to a 20 foot fall hazard while working from a 4 foot by 8 foot job-made temporary work platform which was constructed by superintendent, Matt Boehrer, on the job site, the box was not specifically designed by the lift's manufacturer, and was elevated by a SKYTRAK Model: 6042, powered industrial truck.     No abatement certification or documentation required for this item.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:       a)  Rear Parking Lot - On or about 08/17/2017, workers were exposed to a 20 foot fall hazard while occupying an elevated 4 foot by 8 foot job-made temporary work platform which was moved horizontally and was not attached to the forks of a Skytrak Model 6042 (serial no. 0160052482) powered industrial truck.     No abatement certification or documentation required for this item.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     a)  Rear parking Lot - On or about August 17, 2017, three construction laborers were exposed to a 20 foot fall from a temporary work platform which had not been provided with standard guardrails to any alternative means of fall protection, including, but not limited to, personal fall protection, nets or similar effective means.     No abatement certification or documentation required for this item.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   a)  Rear Parking Lot - On or about August 17, 2017, construction workers working from a temporary work platform elevated by a powered industrial truck were not provided with training by a qualified person.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
