29 CFR 1926.20(b)(2):   On 7/25/2017, a subcontractor of Happy Roof & Exteriors, LLC was removing and replacing roofing materials from a roof of a commercial building located at 119 East Main Street in Warrenton, Missouri.  Happy Roof & Exteriors, LLC did not conduct an adequate inspection of the job site, materials, and equipment.  During the inspection, employees were exposed to the following hazards:  a)  A subcontractor employee was not wearing eye protection while using a pneumatic nail gun to install sheathing. b)  Several subcontractor employees were not wearing protective helmets while working beneath other employees, which were installing roofing materials directly above them.  The employees were subject to head injuries from possible falling tools and supplies. c)  The project manager, subcontractors and their employees were observed on the roof of the building approximately ten feet above the next lower level without utilizing any fall protection.

They were not utilizing guardrail systems with toeboards, safety net systems, or personal fall arrest systems.
