29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds per employee attached, or were not designed, installed and used: a. Site - On or about August 16, 2017, the anchor point in use by employees for fall protection means was not adequate, in that two fall arrest systems were attached to one anchor point.
