29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) LCDE Production Area Maromatic; On or about August 8, 2017, and times prior, the employer exposed employees to potential amputation hazards in that employees were required to conduct service and/or maintenance on equipment such as but not limited the Maromatic - CYA Pinning Machine without being provided with a machine specific lockout/tagout (LOTO) procedure or alternate procedure to safeguard workers while conducting troubleshooting during routine, repetitive, and integral use of the machine to prevent production stoppage.

29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): a) LCDE Production Area Maromatic; On or about August 8, 2017, the employer exposed employees to amputation hazards in that employees were required to troubleshoot, test or sensor position equipment such as but not limited to the Maromatic - CYA Pinning Machine while the machine was energized. The employer failed to follow the required sequence of actions as set by the standard when equipment is energized for testing or positioning. Employees were not removed from the machine or equipment area prior to the machine being energized.
