29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) 2nd floor, elevator shaft: Two employees were removing the elevator shaft cover at the 2nd level. The employer did not ensure the use of fall protection. An employee fell approximately 23 feet to the cellar level. On or about 08/18/17.

29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) 2nd floor, elevator shaft: Cover for elevator shaft opening was not secured so as to prevent accidental displacement. A ledger beam was displaced out of the beam pocket when removing foam filler around the beam. An employee fell approximately 23 feet. On or about 08/18/17.
