29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  On or about August 18, 2017, Elite Residential Roofing LLC, as controlling employer, did not ensure that employees of Mauricio Mecgar, subcontractor, were protected from overhead falling object hazards while working on lower level roofs and on the ground below roofing operations, without wearing a hard hats.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a)  On or about August 18, 2017, Elite Residential Roofing LLC, as controlling employer, did not ensure that employees of Mauricio Mecgar, subcontractor, were protected from eye injuries while using pneumatic nail guns, and while working near pneumatic nail guns, without eye protection. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  On or about August 18, 2017, employees were exposed to falls hazards while roofing a house without properly using fall protection.  The fall hazard ranged from 11 feet from the lower level roofs, to 20 feet and greater on the upper level roofs.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
