29 CFR 1910.95(g)(5)(ii): Where mobile test vans were used to meet the audiometric testing obligation, the employer did not obtain a valid baseline audiogram within 1 year of an employee's first exposure at or above the action level and where baseline audiograms were obtained more than 6 months after the employee's first exposure at or above the action level, employees did not wear hearing protectors for any period exceeding 6 months after first exposure until the baseline audiogram was obtained: 163 Pioneer Dr, Leominster MA, Injection Molding Department, on or about 8/22/17: (a) The employer did not obtain a baseline audiogram within 1 year of a floor person's first exposure to occupational noise above the action level (AL). The floor person was exposed to 86 dBA as an 8-hour time-weighted average (TWA) which is greater than the AL of 82.1 dBA for 12-hour shifts. (b) The employer did not ensure that a floor person wore hearing protection while exposed to 86 dBA as an 8-hour TWA which is greater than the AL of 82.1 dBA for 12-hour shifts.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average (TWA) of 85 decibels (action level) in accordance with the requirements of 29 CFR 1910.95(k): 163 Pioneer Dr, Leominster MA, Injection Molding Department: (a) On or about 9/28/17, five machine operators were exposed to occupational noise at 89.8, 88.1, 87.9, 87.7, and 85.8 decibels measured on the A scale (dBA) as an 8-hour TWA, and the employer did not train the employees on the effects of noise on hearing, the purpose and various types of hearing protection, or the purpose of audiometric testing and explanation of the test procedure. The machine operators work 12-hour shifts and the adjusted action level is 82.1 dBA. (b) On or about 8/22/17, the floor person was exposed to 86.0 dBA as an 8-hour TWA, and the employer did not train the employee on the effects of noise on hearing, the purpose and various types of hearing protection, or the purpose of audiometric testing and explanation of the test procedure. The floor person works a 12-hour shift and the adjusted action level is 82.1 dBA.
