29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average (TWA) of 85 decibels (action level) in accordance with the requirements of 29 CFR 1910.95(k):    163 Pioneer Dr, Leominster MA, Injection Molding Department:    (a)  On or about 9/28/17, five machine operators were exposed to occupational noise at 89.8, 88.1, 87.9, 87.7, and 85.8 decibels measured on the A scale (dBA) as an 8-hour TWA, and the employer did not train the employees on the effects of noise on hearing, the purpose and various types of hearing protection, or the purpose of audiometric testing and explanation of the test procedure.  The machine operators work 12-hour shifts and the adjusted action level is 82.1 dBA.    (b) On or about 8/22/17, the floor person was exposed to 86.0 dBA as an 8-hour TWA, and the employer did not train the employee on the effects of noise on hearing, the purpose and various types of hearing protection, or the purpose of audiometric testing and explanation of the test procedure.  The floor person works a 12-hour shift and the adjusted action level is 82.1 dBA.
