29 CFR 1918.51(b): All components of cargo handling gear, including tent gantlines and associated rigging, were not inspected by the employer or a designated person before each use and at appropriate intervals during use. a) 3411 Jourdan Rd., New Orleans, LA; On or about August 19, 2017, the employer failed to ensure a four way bridle was inspected before use. The hooks on the four way bridle were missing the latches.

29 CFR 1918.81(a): Drafts were not safely slung before being hoisted. a) 3411 Jourdan Rd., New Orleans, LA; On or about August 19, 2017, the employer failed to ensure that a forklift was safely slung before being hoisted. As a rigged forklift was being lifted to be placed onto a ship, it fell over and struck/crushed a dockman standing nearby the draft.

29 CFR 1918.81(b): Cargo handling bridles, such as pallet bridles, which are to remain attached to the hoisting gear while hoisting successive drafts, shall be attached by shackles, or other positive means shall be taken to prevent them from being accidentally disengaged from the cargo hook. a) 3411 Jourdan Rd., New Orleans, LA; On or about August 19, 2017, the employer failed to ensure that each hook on a four way cargo handling bridle had a latch.
